New here? Start with these tabs
- Glossary — every acronym you'll see across this dashboard (CoCM, BCM, PHQ-9, Tier 1/2/3, and more).
- Company Goals — the 3-5 things the whole company is working toward this quarter.
- Practice Portfolio — the full roster of active practices, contacts, and current status.
- Handbook — the SOPs for how Provider Engagement work actually gets done.
Reviewed as a leadership team every Monday to check growth against the 4-week plan for the month.

Pulled straight from the calendar every 5 minutes via the live-sync Worker — next 14 days, practice and team meetings only (personal "Office"/"Home" blocks are filtered out).
Two live strategy documents govern current campaign activity: the in-market baseline (running today) and the proposed seasonal/structural additions for the next quarter.

In-market baseline running today
- Weekly/biweekly practice touchpoints — one-page performance summaries at virtual check-ins.
- Monthly in-person visits — full clinical, financial, and referral review for Tier 3 (stable) accounts.
- Proactive outreach — direct patient contact for eligible, unreferred patients; currently reaches 7–8 practices.
- EMR flagging — alerts PCPs to patients matching referral criteria (Dx/Rx/PHQ-9/GAD-7).
- Provider dashboard — real-time engagement/screening view; adoption mixed, under review for retirement or redesign.
- Provider in-services — live training sessions at partner practices.
- Monthly EMR-embedded success stories — individual patient outcome highlights.
Seasonal campaign calendar proposed, Jul 13
- Winter (Jan–Feb) — demand peak. Seasonal affective disorder drives a genuine clinical increase; highest-leverage window for referral infrastructure.
- Summer (Jun–Aug) — capture, not demand. Underlying need doesn't drop, but capture rates fall as patients and staff are away.
- Fall — rebound. Deductibles reset, school physicals cluster; natural on-ramp for renewed push.
- Holiday delay effect produces a predictable January surge worth pre-staffing for.
Q3 funnel mapping
- IDENTIFY — EMR flagging, screener completion pushes
- ENDORSE — champion PCP peer comparison, gold/silver/bronze tiering
- CONVERT — proactive outreach, batch referrals
- RETAIN & PROVE — success stories, clinical-outcome practice snapshots
Active named campaigns / initiatives
- CHS Network Expansion — Phase 1: Mercy Comprehensive Care Center, Trinity–Bowmansville, Trinity–North Forest. Champion PCPs identified; clinic-manager informational session in planning.
- Allied Physicians relationship repair: resolved 6/30 — proactive-outreach texting to patients whose PCPs had declined was stopped, ops notified, no further referrals affected.
- Sunrise / Family Care / Southgate / Southtowns / Associated Physicians: new-onboarding or reengagement push, all currently Tier 1.

Add a new document
Add a link to a new SOP, template, or reference doc — paste the Google Drive link once it's uploaded there. This adds it to the list below for this session; ask Claude to bake it into the next site update to make it permanent.
Every tracker, SOP, playbook, and template the team currently works from — identified from Drive so nothing lives in a separate silo. Flag anything missing and I'll add it.
