- Referral concentration: 7 of 14 partners (58%) drive 84% of total referral volume.
- Mature-account decline: PCWNY (30-day rate ↓55%), Northtowns (↓56%), Madison Medical (↓78%), Dr. Beney (↓31%) are established accounts quietly slipping.
- Accelerating accounts: CHS–St. Vincent (+233%), Lakeshore (+340%), Dr. Laurri (+200%) — newer/smaller accounts picking up.
- The provider dashboard is solving a visibility problem, not the actual referral barrier — office managers say they'd open it at most weekly.
- Proactive outreach is the most clinically credible campaign but reaches only a fraction of eligible patients and tapers without a forced review cadence.

- Weekly/biweekly practice touchpoints — one-page performance summaries at virtual check-ins.
- Monthly in-person visits — full clinical, financial, and referral review for Tier 3 (stable) accounts.
- Proactive outreach — direct patient contact for eligible, unreferred patients; currently reaches 7–8 practices.
- EMR flagging — alerts PCPs to patients matching referral criteria (Dx/Rx/PHQ-9/GAD-7).
- Provider dashboard — real-time engagement/screening view; adoption mixed, under review for retirement or redesign.
- Provider in-services — live training sessions at partner practices.
- Monthly EMR-embedded success stories — individual patient outcome highlights.
Seasonal campaign calendar proposed, Jul 13
- Winter (Jan–Feb) — demand peak. Seasonal affective disorder drives a genuine clinical increase; highest-leverage window for referral infrastructure.
- Summer (Jun–Aug) — capture, not demand. Underlying need doesn't drop, but capture rates fall as patients and staff are away.
- Fall — rebound. Deductibles reset, school physicals cluster; natural on-ramp for renewed push.
- Holiday delay effect produces a predictable January surge worth pre-staffing for.

- Allied Physicians relationship repair: resolved 6/30 — proactive-outreach texting to patients whose PCPs had declined was stopped, ops notified, no further referrals affected.
- CMP Provider Dinner (Remington Tavern): relationship-building event for CMP-network physicians and office managers.
- Sunrise / Family Care / Southgate / Southtowns / Associated Physicians: new-onboarding or reengagement push, all currently Tier 1.
